The Social Worker collaborates with an interdisciplinary team to manage complex discharge planning and ensure timely patient transitions to post-hospital settings. They provide emotional support, supportive counseling, and coordinate with external agencies to meet patients' biopsychosocial needs.
Candidates must hold a Master's degree in Social Work and an active Missouri LMSW or LCSW license. A minimum of 2 years of hospital experience and in-depth knowledge of community resources are required.

The Social Worker will work in collaboration with members of the interdisciplinary team to enhance the cost-effective utilization of resources by identifying and initiating complex discharge planning to accomplish the timely transition of patients to post-hospital settings.
The ideal candidate must be highly detailed and goal-oriented, with superior analytic, critical-thinking and time-management skills. They must have a positive and empathetic personality with the ability to work effectively with others as part of a multidisciplinary team in a complex care environment. It is also vital that they have excellent communication and interpersonal skills and consistently display a professional attitude.
