Position Title

ACT Licensed Practical Nurse (LPN) Department: Assertive Community Treatment (ACT) Reports To: ACT Registered Nurse (RN), ACT Team Lead, and Director of Behavioral Health Services

Position Summary

The ACT Licensed Practical Nurse (LPN) provides community-based nursing services as part of a multidisciplinary Assertive Community Treatment (ACT) Team. The LPN assists individuals with serious mental illness, co-occurring disorders, and complex healthcare needs by providing medication support, health monitoring, education, and coordination of care in community settings.

• Essential Duties and Responsibilities
• Administer medications as prescribed and within scope of practice.
• Monitor medication effectiveness, side effects, and adherence.
• Assist with medication education and self-management training.
• Conduct routine health assessments and monitor vital signs.
• Observe and report changes in physical or mental health status.
• Provide services in homes, shelters, hospitals, and community locations.
• Accompany individuals to medical appointments when clinically indicated.
• Collaborate with ACT team members to implement treatment plans.
• Participate in treatment team meetings and case consultations.
• Complete documentation accurately and timely within the electronic health record.

Qualifications

• Graduate of an accredited Practical Nursing Program.
• Current unrestricted Virginia Licensed Practical Nurse (LPN) license.
• Current CPR certification required.
• Minimum one year of nursing experience preferred.
• Behavioral health or community mental health experience preferred.

• Participation in after-hours and on-call coverage as assigned.
