RN Case Manager

Transitions At Home, is currently seeking to hire a compassionate and experienced Full-Time RN Case Manager to join our growing home health team. This position offers the opportunity to provide personalized care in a home setting, ensuring clients receive the highest quality of service and support throughout their recovery journey. What does a Transitions at Home RN Case Manager do? Transitions At Home Registered Nurse Case Managers provide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the client and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours

• Full-Time
• Included: Company Car and Gas Card
• 40 hours per week 8:00 am - 4:30pm
• Monday through Friday
• No Night Shifts (also known as PM and NOC Shifts)
• Limited on-call rotation

• Active Wisconsin Registered Nurse License
• Two (2) years’ nursing experience, 1 year in Home Health (preferred)
• CPR Certification
• A valid Wisconsin Driver's License and Car Insurance
