The Community Health Worker provides personalized, community-based care to pregnant and postpartum individuals and their infants. Responsibilities include conducting home visits, identifying social determinants of health, and connecting clients with essential community resources.
Candidates must have a high school diploma and a valid Ohio Driver's License, with CHW certification required within the first year or already held. Experience in public health, OBGYN, or social work is preferred, along with proficiency in Electronic Health Record systems.

Now hiring: Community Health Worker (Beautiful Beginnings Section) Grant-funded
The Community Health Worker (CHW) will work closely with individuals and their infant(s) in the community during pregnancy, the postpartum period, and/or up to the child’s third birthday. The CHW will work in tandem with a Public Health Nurse to provide personalized, community-based care to these clients. The CHW will assist clients in finding community resources which reduce their barriers to care and help ensure that the client is knowledgeable about their health and the health of their infant(s). This position is grant funded through a Community Partnership Grant.
