The Care Coordinator manages a caseload of complex patients to link them with community resources addressing social determinants of health. Responsibilities include scheduling appointments, managing referrals, and collaborating with clinical teams to monitor patient progress.
Requires a high school diploma and at least two years of experience in healthcare, behavioral health, or social services. A valid CA driver's license and insurance are mandatory, while medical certifications and bilingual skills are preferred.

The Care Coordinator will coordinate care for complex patients with mental illness, substance use disorders, or chronic conditions to navigate and link with community resources that address social determinants of health (SDOH). This role will provide care coordination and connection to services and social supports for ECM patients, including appointment scheduling and referral management. Additionally, this position will require travel to other Neighborhood sites as needed to attend the designated meetings as well as training for staff members.
**This is a hybrid position; on site expectations are dependent on performance. **
$24.95 - $34.93 per hour, depending on experience
The posted salary range reflects the designated pay grade for this position. While this range represents the broader classification of the role, actual compensation will be based on several factors, including but not limited to: the candidate’s overall knowledge, skills, and experience, market data and industry benchmarks, internal equity within the organization, Budgetary considerations and organizational needs. As a result, placement within the range is not guaranteed, and the full pay grade range may not be utilized.
