- Summary The Fee for Service Social Worker is responsible for providing individual and group counseling and direct service to children, teens, adults and their families.
- Services include screening and assessment, treatment planning, provision of counseling/psychotherapy and crisis intervention.
- The Fee for Service Social Worker is a member of the treatment team and collaborates with other team members.
- Essential Functions: Provide direct patient care to clients using a variety of social work modalities in order to help them maximize their level of functioning.
- Maintain appropriate professional patient care documentation in the agency selected Electronic Health Record (EHR), including client assessment and individual notes in compliance with all city, state and federal standards of practice.
- Facilitate the client’s utilization of available support systems and community resources to meet needs identified in clinical evaluation or treatment planning.
- Meet as requested/regularly with supervisor to discuss achievement of program targets, client goals, quality of client care and reporting requirements.
- May conduct consultations with ACS Child Protective Specialists or other internal or external staff on concerns they may have with clients.
- May attend case reviews and develop next steps for the families and clinicians Meets data metric requirements, quality of care expectations using traditional social work interventions, or evidence-based models.
- Performs other related duties as assigned.
Minimum Qualifications
Master’s Degree in Social Work, from an accredited School. LMSW or LCSW certification Bilingual in one or more of the following: Spanish, Mandarin, Cantonese, Bengali and others, strongly preferred. Proficiency in Microsoft Office. Ability to plan and carry out assignments independently. Ability to prioritize, adhere to timelines and multi-task.
Schedule
- Work may be performed during weekday evening hours and/or on Saturday’s and is limited to 14 hours weekly.
- Physical demands: While performing the duties of this job, the employee is occasionally required to walk, sit for long period of time; use hands to finger, handle, or feel objects, tools, or control; reach with hands and arms; balance; stoop; talk or hear. Employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.
