The RN Admissions manages patient placement by coordinating admissions, discharges, and transfers within the facility. They collaborate with providers and the Command Center to determine appropriate inpatient or observation status and oversee discharge planning.
Requires an Associate's Degree in Nursing and at least 2 years of acute care experience. Must hold a valid California RN license and current BLS and ACLS certifications.
The RN Admissions is responsible for receiving admission, discharge and transfer information to facilitate patient placement in appropriate bed assignments. The ability to assess chart reviews, case management and patient flow, while working collaboratively with providers, is essential. Responsible for various aspects of acute care coordination through intake, admission and transition. In collaboration with the Command Center, the RN Admissions will assist in identifying patients appropriate for inpatient or observation admission status. This position oversees, coordinates and communicates discharge planning services for patients through the healthcare continuum. Collaborates with the multidisciplinary team on discharge processes and serves as a liaison on community facilities and resources.
