Pay Range $30.70 - $46.05 The purpose of this position is to help achieve the vision of Advocate Home Health Services by performing the duties of this position in a cost effective, results oriented and customer focused manner. This position is a critical component of the Unit team and as such meeting individual performance expectations and contribution to team performance goals is required. Assesses the home care patient's psycho-social needs as related to their family background and environment as it impacts the medical condition and the prognosis for positive outcome; formulate and implement an effective treatment program to meet those needs; and coordinate with all disciplines regarding the patient's social welfare.

Conditions: Extensive travel within geographical boundaries in all extremes of weather and on all types of roads including expressways. Must be able to provide/arrange own transportation to and from patients homes and branch office. Ability to work rotating schedules as needed. Requires adherence to scheduled days and hours of work based on commitment to patient/client and work load assigned. If position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06/01/2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.
