Nurse Navigator - Admissions RN
Schedule
Four 10-hour shifts, including rotating weekends and holidays across a 7-day schedule.

What You’ll Be Doing
- As a Nurse Navigator in our outpatient setting, you’ll be the first clinical connection for patients and families beginning hospice services. You'll handle assessments, coordinate care, and support patients in the comfort of their own homes or other care facilities.
- Collaborate with external medical professionals and referral sources to organize and complete admissions into hospice care
- Build positive relationships with referral partners to streamline the admission process and increase awareness of available services
- Coordinate with outside vendors, such as transport companies or DME providers, to ensure smooth transitions into care
- Complete initial assessments and plan of care documentation during admission visits (typically around 3.5 – 4 hours)
- Clearly document eligibility for hospice based on clinical criteria such as:
- Uncontrolled pain
- Advanced disease symptoms requiring increased support
- Declining condition consistent with hospice guidelines
- Provide education and support to caregivers and family members, including:
- What to expect at end-of-life
- Medication guidance and symptom management
- Recognizing changes in condition
- Perform real-time documentation to ensure accuracy and compliance
- Coordinate care with our Interdisciplinary Team (IDT), including physicians, nurse practitioners, RNs, social workers, chaplains, and CNAs
- Communicate and carry out admission orders from NPs and MDs
What We’re Looking For
- Graduate of an accredited nursing program (AD, Diploma, or BSN)
- Current RN license in North Carolina
- At least 2 years of nursing experience – hospice or home care preferred
- Strong communication and documentation skills
- Comfortable spending long amounts of time in your car – you’ll be driving a lot.
This department operates 7 days per week. The schedule for this role is four 10-hour shifts per week.
