Perform initial and ongoing assessments of patients' physical and psychosocial needs to implement individualized care plans. Provide education on disease processes and end-of-life care while supervising paraprofessional staff.
Candidates must hold a current Michigan nursing license in good standing. A minimum of one year of experience in home health and hospice care is required.
