Evaluate and implement individualized speech therapy treatment plans for residents in skilled nursing and specialized ventilator care settings. Collaborate with an interdisciplinary medical team to treat swallowing, communication, and cognitive disorders while maintaining regulatory documentation.
Requires a Master's degree in Speech-Language Pathology and a current Wisconsin license or eligibility for licensure. Experience in post-acute care, dysphagia, or ventilator populations is preferred but not required.
Evaluate, develop, and implement individualized speech therapy treatment plans. Assess and treat swallowing, communication, cognitive, and voice disorders. Collaborate with physicians, nursing staff, respiratory therapy, dietitians, and interdisciplinary team members. Complete accurate and timely documentation in accordance with Medicare, Medicaid, and regulatory guidelines. Participate in care conferences, discharge planning, and interdisciplinary team meetings. Provide patient, family, and caregiver education. Support quality outcomes and person-centered care initiatives.
Master's degree in Speech-Language Pathology. Current Wisconsin Speech-Language Pathologist license or eligibility for licensure. Experience in skilled nursing, post-acute care, dysphagia management, tracheostomy care, or ventilator populations is preferred but not required. Strong communication, organization, and clinical decision-making skills.
