What You'll Do As a Hospice LPN, you will provide direct patient care under the supervision of a Registered Nurse and in accordance with physician orders and individualized plans of care.
• Responsibilities include: Providing skilled nursing care to hospice patients in their homes and care facilities Monitoring and reporting changes in patient condition Assisting with symptom management and patient comfort Educating patients, families, and caregivers regarding care needs Collaborating with the interdisciplinary hospice team Maintaining accurate and timely clinical documentation Participating in on-call, holiday, and weekend rotations as assigned Supporting patients and families with professionalism, empathy, and respect Qualifications Current Oklahoma Licensed Practical Nurse (LPN) license Hospice, home health, or related nursing experience preferred Strong clinical assessment and communication skills Ability to work independently while collaborating effectively with the care team Valid driver's license, reliable transportation, and current auto insurance Ability to travel throughout the service area Why Join Haven?
