Acts as the primary consumer advocate by coordinating internal and external services and developing individualized service plans. Responsibilities include conducting assessments, clinical documentation, and providing HIV/VH testing and counseling.
Requires a bachelor's degree in Social Work, Psychology, Addiction Counseling, Criminal Justice, or a related field. Two years of supervised case management experience is preferred.
The person in this position will be responsible to act as the primary consumer advocate for the program assigned. The person in this position keeps abreast of progress, and coordinates all services delivered to the consumer and family internally and through external referral. Essential Job Functions: Administer assessments as needed Develop Service Plans in collaboration with the consumer and family and Conduct monthly Service Plan reviews Document all clinical services and activities in accord with licensing, Medicaid, and Joint Commission standards and agency policy and procedure Conduct individual case management sessions Provide vocational education and skills building to the consumer as needed Perform activities on behalf of the consumer and family, including attendance at external appointments (e.g., court appearances, school staffing, housing assistance etc.), as needed Provide referral and linkage for the consumer and family, as needed Coordinate consumer and family care with the treatment team and with external providers Arrange transportation for consumers, as necessary Conducts HIV/VH test using OraSure or OraQuick and conduct pre/post counseling. Provides education about safer sex practices and substance use prevention. The ability to identify and evaluate an individual’s strengths, barriers, problems, and needs for the development of a Service Plan and the ability to evaluate the impact of substance use, mental health problems and HIV/VH/STD’s on the client’s functioning in major life areas. Ability to communicate effectively with adults (18+) to motivate them to change high-risk behaviors and/or to utilize available resources. Understanding of high risk behaviors related to HIV/VH/STD infection. The ability to identify the needs of a client and family that cannot be met by the agency and to assist the client/family to utilize the support systems and community resources. QualificationsEssential Qualifications: Education: Graduation from an accredited bachelors program with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or related field is required Experience and Competencies: Two years of previously supervised work experience in providing case management services is preferred
