The Resource Specialist provides person-centered information and assistance to older adults and adults with disabilities regarding available resources and services. They are responsible for conducting screenings to determine eligibility for Wisconsin's Long Term Care Programs and completing required documentation.
Candidates must possess a Bachelor's degree in human services, social work, or a related field and have at least one year of experience working with older adults or adults with disabilities. A valid driver's license and the ability to obtain Functional Screener and Options Counseling certification are required.
The Resource Specialist is responsible for assisting older adults and adults with a disability, their families and caregivers by providing person-centered information and assistance regarding resources and services, completes long term care options counseling and determines eligibility for Wisconsin’s Long Term Care Programs. Position works closely with internal teams and community partners to develop collaborative relationships to ensure proper referrals, working together to help individuals. This position is based in the Wausau Office. Occasional travel throughout the ADRC-CW service region for meetings and case assistance is required. Examples of Work Performed: The following duties are typical of this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required or assigned. Provide accurate information to individuals regarding available resources. Assist individuals to access services to meet their needs. Conducts screening to determine eligibility for Wisconsin’s Long Term Care Programs. Provide assistance to individuals with Long Term Care Medicaid applications. Complete enrollment and disenrollment for Wisconsin’s Long Term Care programs. Complete required documentation into client tracking system. Refer customers to other agencies and services when appropriate Complete Outreach to target populations and community agencies Develop and expand job related skills and knowledge by attending training. Participate in agency committees and community groups Complete and submit all required paperwork timely and accurately. Maintains regular and predictable attendance. Performs related work as required.
Bachelor’s degree in human services, Social Work or closely related field. At least one (1) year working with older adults or adults with a disability. A valid driver's license and a driving record that meets County standards. Knowledge, Skills, and Abilities: Ability to obtain Functional Screener and Options Counseling certification within 3 months of date of hire Ability to work effectively with older adults and adults with disabilities Ability to conduct assessments of customer needs through telephone or in person interviews Skills in interviewing individuals in a one on one or group setting. Excellent interpersonal, verbal and written communications skills. Ability to develop and maintain rapport with community partners. Ability to communicate detailed and accurate information to individuals and groups. Knowledge of community resources that assist older and disabled adults. Adhere to program and position standards Ability to research, analyze and resolve problems. Ability to listen and respond in a nonjudgmental and respectful manner. Ability to work independently and exercise judgment in accordance with established procedures Ability to travel to individuals’ homes and community locations. Ability to be organized, multitask and meet deadlines. Commitment to ADRC-CW mission, contributing to a positive work culture that fosters teamwork and excellent customer service. Adhere to confidentiality requirements. Knowledge of and commitment to adopting a trauma informed approach to practice Skills in utilizing a computer using a variety of software, email and internet.
