The ACH RN provides high-acuity virtual care to hospitalized patients in their homes using telemedicine technology. Responsibilities include patient assessment, medication verification, and collaborating with a multidisciplinary team in a Command Center.
Candidates must be a graduate of a nursing program with a current RN license and BLS competency for Florida/Arizona. A bachelor's degree is required within a specific timeframe for associate degree holders, and one year of RN experience is preferred.
Advanced Care at Home (ACH) is a virtual care inpatient nursing unit that follows the hospital at home model of care. ACH is an implementation of one of three initial platform ventures and is a new model of high-acuity care for the home. ACH uses telemedicine technology to remotely monitor and provide care via videoconferencing for hospitalized patients in their home setting instead of, and in addition to, the established clinical setting across the Mayo Clinic Enterprise. A team of nurse practitioners, physician assistants, registered nurses, case managers and pharmacists work collaboratively while located remotely in an ACH Command Center.
The ACH RN performs a variety of skills including patient assessment, medication verification, patient education, patient acquisition, and discharge. The virtual nurse collaborates with the multidisciplinary care team to coordinate care needs and therapies in the patient home. ACH RNs work extensively within 'Medically Home' which has been chosen as Mayo Clinic Platform's partner to provide the technology that orchestrates the delivery of these services into the home and supporting infrastructure. In addition to working on site via telemedicine, ACH RN's provide coverage on inpatient units as needed.
