The Case Manager provides outreach to homeless individuals in Staten Island, completing assessments and facilitating referrals for medical and psychological services. They assist clients in securing interim and permanent housing while maintaining accurate program statistics.
Candidates must possess a High School Diploma or GED and a valid driver's license. The role requires the ability to drive an outreach van to escort clients to necessary services.
Connect with homeless people living on Staten Island completing assessments and appropriate referrals including psych evaluations, TB and HIV testing, detox, etc. Assists clients in filling out forms for entitlements, etc. Complete psychosocial and social history when applicable. Counsel clients leading to interim, transitional and permanent housing or shelter placements Complete all paperwork as well as accurate timely statistics as required by the program Respond in a timely way to calls from the community regarding homeless individuals in all areas of Staten Island Drive and escort clients to the necessary services they require (ESU, detox, etc) using the Outreach van Participate in weekly supervision, case conferences and staff meetings QualificationsHigh School Diploma Valid Drivers License
