Summary: The Case Manager will work within our Outreach and Housing Placement Program, providing outreach to homeless people living in or near the North Shore area of Staten Island and responding to community calls regarding outreach to other areas of Staten Island.

Responsibilities

Connect with homeless people living on Staten Island completing assessments and appropriate referrals including psych evaluations, TB and HIV testing, detox, etc. Assists clients in filling out forms for entitlements, etc. Complete psychosocial and social history when applicable. Counsel clients leading to interim, transitional and permanent housing or shelter placements Complete all paperwork as well as accurate timely statistics as required by the program Respond in a timely way to calls from the community regarding homeless individuals in all areas of Staten Island Drive and escort clients to the necessary services they require (ESU, detox, etc) using the Outreach van Participate in weekly supervision, case conferences and staff meetings QualificationsHigh School Diploma Valid Drivers License
