Provide skilled, one-on-one nursing care to patients in their homes and develop individualized care plans. Responsibilities include documenting patient observations and collaborating with interdisciplinary teams to ensure regulatory compliance.
Candidates must hold a current Colorado RN license, CPR certification, and a valid driver's license with a reliable vehicle. Home health experience is preferred, and applicants must pass pre-employment screenings.
