Occupational Therapist (OT) | All Heart Home Care | Murfreesboro Area | Full-Time All Heart Home Care is seeking a compassionate and skilled Occupational Therapist (Full-Time) to join our home health team in the Murfreesboro area. This role offers a consistent schedule while providing personalized, one-on-one care to patients in the comfort of their homes. Key Responsibilities: Perform in-home evaluations to assess patient needs and develop individualized treatment plans Provide skilled occupational therapy services to improve daily living skills, strength, coordination, and independence Monitor patient progress and adjust treatment plans as appropriate Educate patients and caregivers on adaptive techniques, home safety, and therapeutic exercises Complete accurate and timely clinical documentation in compliance with company policies and state regulations Coordinate care with physicians, nurses, case managers, and other members of the healthcare team Travel to patient homes within assigned service areas
Degree in Occupational Therapy from an accredited program Active Occupational Therapist license in the state of Tennessee Current CPR certification Valid driver's license, reliable transportation, and current auto insurance Strong clinical assessment and communication skills Ability to work independently and manage time effectively Previous home health experience preferred but not required Work Schedule: Full-Time Consistent caseload based on patient volume and service area About All Heart Home Care: At All Heart Home Care, we are committed to providing high-quality, compassionate care to patients in their homes.
