Home Health Physical Therapist PRN

• The Home Health Physical Therapist plays a critical role in delivering personalized rehabilitative care to patients within their own homes, promoting recovery, mobility, and independence.
• This position requires assessing patients' physical conditions, developing tailored treatment plans, and implementing therapeutic interventions to address a variety of musculoskeletal and neurological conditions.
• The therapist collaborates closely with patients, families, and interdisciplinary healthcare teams to ensure comprehensive care and optimal outcomes.
• By providing education and support, the therapist empowers patients to manage their health and prevent further injury.
• Ultimately, this role contributes significantly to improving patients' quality of life by facilitating functional improvement in a comfortable and familiar environment.

As a Physical Therapist, you will

• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
• Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Applies concepts of infection control/universal precautions in performing patient care
• Assure continuity of quality patient care delivered with appropriate documentation
• Provide effective communication to patient/family, team members and other health care professionals
• Promote agency philosophy and administrative policies
• Maintain and complete all documentation within company stated guidelines.

Requirements

• Successful completion of accredited physical therapy program and state licensure exam.
• Valid Physical Therapy license
• 1 year of home health experience preferred
• CPR certification
• Valid Driver’s license
• Valid Auto Insurance
• HomeCare HomeBase experience preferred

• Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
• Must be able to lift, carry, pull, or push up to 100 lbs..
