Conduct initial patient assessments in the home setting
Develop and implement individualized treatment plans
Coordinate care with physicians and healthcare teams
Promote mobility, independence, and healing through compassionate, hands-on care
Use our proven case management model to guide patient outcomes and support families What You'll Need:
Active Florida Physical Therapist license
Minimum 2 years of post-graduate experience preferred
Orthopedic or home health experience is a plus
