The LPN is responsible for administering medications, monitoring resident health, and assisting with daily personal care needs. They must maintain accurate records and collaborate with the healthcare team to ensure comprehensive resident well-being.
Candidates must hold a current and valid LPN license in Illinois and be available to work one weekend per month. Experience in assisted living or long-term care settings is preferred.

We are seeking a dedicated and compassionate PRN Licensed Practical Nurse (LPN) to join our team. The LPN will be responsible for providing high-quality care to our residents, ensuring their health and well-being, and assisting with daily activities. The ideal candidate will have a strong commitment to patient care and a desire to work in a supportive, team-oriented environment.
Administer medications and treatments as prescribed by physicians Monitor residents’ health and report any changes to the healthcare team Assist and provide direction to residents regarding their personal care needs, including grooming, hygiene, dressing, nutrition, housekeeping, etc. Maintain accurate and detailed records of care and treatment Collaborate with other healthcare professionals to ensure comprehensive care Educate residents and their families on health conditions and care plans Ensure a clean, safe, and supportive environment for all residents Other duties that may become necessary, or as directed by your supervisor or Nurse Manager This position-shifts will vary based on needs.
Current and valid LPN license in Illinois Required to work 1 weekend per month Experience in an assisted living or long-term care setting preferred Strong communication and interpersonal skills Ability to work in a team-oriented environment Compassionate, patient, and empathetic approach to care Ability to handle multiple tasks and prioritize effectively We are an equal opportunity employer.
