Coordinates services based on daily staffing’s with other clinical team members, provides experience, strength, and hope to individuals, their families, and significant others by sharing their recovery experience and knowledge of the behavioral health system and available community resources to promote recovery and self-improvement. Key Responsibilities Provides ongoing assessment and feedback as to their progress in relation to their service plan, regardless of the consumer’s current location. Meets with members in the community in accordance with established policies and procedures and clinical protocols to provide supportive counseling skill development and other support services as needed. Works with the assigned members of the ACT l team to ensure the timely development and implementation of the individual’s service plan (including all necessary assessments) and their crisis plan. Facilitates groups appropriate to scope, experience and training. Assists members and families in accessing additional needed services and obtaining entitlements, including identification of funding services. Responsible for transporting members to necessary services, as outlined in their service plan, as needed.
Must be 21 years or older Requires a High School Diploma, or GED 1-4 years of behavioral health experience An Associates degree and 1-2 years of relevant experience in a behavioral health setting A Bachelors degree plus 1 year of relevant experience in behavioral health Previous experience working as a Peer Support Specialist is highly preferred. Peer Certification required upon hire or within 30 days of starting. Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire AZ valid Driver’s License, vehicle with state minimum insurance coverage and able to meet Copa Health company driving requirements and safety sensitive requirements.
