The Occupational Therapist is responsible for assessing and evaluating patient care needs related to functional status, activities of daily living, and fine motor coordination. They develop and implement therapy plans of care in coordination with physicians and other healthcare team members.
Candidates must hold a current Occupational Therapy licensure in the state of practice and possess a valid driver's license with a dependable vehicle. CPR certification and demonstrated ability to work independently are preferred.
As the Occupational Therapist in Home Health, you will be responsible for assessment and evaluation of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions.
Provides service within the scope of practice as defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team Evaluates the patient’s functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consults with the physician in the development of the therapy plan of care Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Current Occupational Therapy licensure in state of practice Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation
Current CPR certification or ability to complete within 90 days of hire Demonstrated ability to manage multiple tasks simultaneously Demonstrated ability to work independently Proven excellent communication, writing, and organizational skills
Pay Range $80,220 $120,330 annual total cash target pay $38.57 $57.85 per visit point $46.28 $69.42 hourly rate
