• Perform comprehensive nursing assessments and develop individualized plans of care.
• Deliver skilled nursing interventions, including medication management, wound care, treatments, and patient education.
• Monitor patient conditions and communicate changes with physicians and interdisciplinary team members.
• Educate patients and caregivers to promote safety, independence, and successful management of health conditions at home.
• Utilize critical thinking skills to coordinate care and identify patient needs.
• Accurately document assessments and services using electronic medical records.

Qualifications

Current New Jersey Registered Nurse (RN) license in good standing. Minimum of one year of nursing experience, preferably in home health. OASIS experience and Start of Care (SOC) experience preferred. Comfortable using electronic medical records; HCHB experience is a plus. Strong assessment, communication, organizational, and time management skills. Compensation
