Physical Therapist

• As a Physical Therapist, you will play a critical role in helping patients recover from injuries, manage chronic conditions, and improve their overall physical function and mobility.
• You will assess patients' physical abilities, develop personalized treatment plans, and implement therapeutic interventions to promote healing and prevent further injury.
• Your work will involve collaborating with healthcare professionals to ensure comprehensive patient care and adapting treatments based on patient progress and feedback.
• You will also educate patients and their families on exercises, injury prevention, and lifestyle modifications to support long-term health.
• Ultimately, your efforts will contribute to enhancing patients' quality of life and enabling them to regain independence in their daily activities.

As a Physical Therapist, you will

• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
• Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Applies concepts of infection control/universal precautions in performing patient care
• Assure continuity of quality patient care delivered with appropriate documentation
• Provide effective communication to patient/family, team members and other health care professionals
• Promote agency philosophy and administrative policies
• Maintain and complete all documentation within company stated guidelines.

Requirements

• Successful completion of accredited physical therapy program and state licensure exam.
• Valid Physical Therapy license
• 1 year of home health experience preferred
• CPR certification
• Valid Driver’s license
• Valid Auto Insurance
• HomeCare HomeBase experience preferred

• Physical Demands
• Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods.
• Must be able to lift, carry, pull, or push up to 100 lbs..
