Directs and provides physical therapy services to patients in their homes based on physician orders. Responsibilities include evaluating patient conditions, developing care plans, and instructing patients and caregivers on medical equipment.
Must be a graduate of an accredited Physical Therapy program with a valid NYS Department of Education license. Requires at least one year of practical experience as a licensed physical therapist.
Role and Responsibilities
A qualified professional, who directs, supervises, evaluates and provides physical therapy services to patients in the home, following physician’s orders.

The Physical Therapist is primarily in a non-office setting and may be exposed to outdoor conditions.
• Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.
