Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas. PACE Program The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity. Speech-Language Pathologist Plays a vital role in helping older adults maintain their communication abilities, swallowing function, and overall independence. Working as part of an interdisciplinary care team that includes physicians, nurses, therapists, social workers, and other healthcare professionals, the SLP evaluates and treats speech, language, cognitive, voice, and swallowing disorders that may result from conditions such as stroke, dementia, Parkinson's disease, or other neurological illnesses. The Speech-Language Pathologist develops individualized treatment plans, provides therapy services, educates participants and caregivers on effective communication and safe swallowing techniques, and recommends adaptive strategies or equipment to enhance daily functioning. Through collaboration with the care team and ongoing assessment of participant needs, the SLP helps improve quality of life while supporting Midland Care's mission of enabling older adults to remain safely and independently in their homes and communities. QualificationsRequired Master's degree in Speech-Language Pathology from an accredited program Current Kansas Speech-Language Pathologist license (or ability to obtain prior to employment) Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association or eligibility to obtain certification Current CPR/BLS certification or ability to obtain upon hire Valid driver's license, reliable transportation, and acceptable driving record for travel to participant homes and community settings Strong assessment, treatment, documentation, and communication skills Ability to work independently and collaboratively within an interdisciplinary healthcare team Proficiency with electronic medical records and basic computer applications Preferred Previous experience working with geriatric populations Experience treating swallowing disorders (dysphagia), cognitive-communication disorders, and neurological conditions such as stroke, dementia, and Parkinson's disease Prior experience in PACE, home health, hospice, rehabilitation, skilled nursing, or other community-based healthcare settings Knowledge of Medicare, Medicaid, and PACE regulations Bilingual communication skills Experience participating in interdisciplinary care planning and team-based healthcare models Home health or community-based therapy experience preferred What’s in it for you? 403(b) ERISA Retirement Plan On-site education center to provide professional development and CEU credits Free Employee Assistance Program (EAP) for employees and dependents Uniform Allowance Program Apply Today! Requirements
