The pharmacist will independently perform sterile and non-sterile compounding in compliance with USP standards while verifying medication orders for accuracy. They will also collaborate with interdisciplinary care teams to monitor patient therapy and educate caregivers on safe medication administration.
Candidates must hold a PharmD or B.S. in Pharmacy and an active Arizona Pharmacist license. A minimum of 2-3 years of experience is required, with at least one year specifically in home infusion, specialty pharmacy, or sterile compounding.

Required: Doctor of Pharmacy (PharmD) or B.S. in Pharmacy Active, unrestricted Arizona Pharmacist license Home infusion, specialty pharmacy, or sterile compounding experience 2–3+ years of pharmacist experience, with at least 1 year in a home infusion, specialty pharmacy, or sterile compounding setting Hands-on proficiency with sterile compounding techniques and USP 797 and USP 800 compliant environments Ability to independently perform compounding, calculations, data entry, and pharmacy workflow tasks Proficiency with pharmacy software and data entry systems Strong organizational and time-management skills with the ability to prioritize effectively in a fast-paced environment Strong communication and patient-facing skills Ability to participate in on-call rotation with a 30-minute response time and holiday rotation as scheduled Preferred: BCSCP certification Experience with IV antibiotics, TPN, or specialty biologics in a home infusion setting Experience supervising or directing pharmacy technicians Experience in a collaborative, interdisciplinary or home health setting Arizona Home Care is an Equal Opportunity Employer.
