The Social Services Assistant supports the Social Services Director in coordinating resources, advocacy, and psychosocial support for residents. Key duties include assisting with admissions and discharge planning, maintaining documentation, and connecting families with community resources.
Preferred experience in social services, healthcare, or skilled nursing with knowledge of discharge planning and resident advocacy. Candidates must possess strong interpersonal skills and proficiency with basic computer systems for documentation.
The Social Services Assistant supports the Social Services Director in ensuring residents receive the resources, advocacy, and support necessary to maintain the highest possible quality of life. This position plays a key role in coordinating services, assisting with discharge planning, maintaining documentation, and helping residents and families navigate healthcare and community resources. Key Responsibilities Resident Support & Advocacy Assist residents in achieving the highest practicable level of independence, self-care, and psychosocial well-being. Provide support to residents and families coping with illness, disability, transitions in care, and other life challenges. Help identify residents' social and emotional needs and assist in developing appropriate service plans. Maintain regular social services progress notes and documentation. Care Coordination Collaborate with administration, nursing, rehabilitation, and other departments to coordinate resident services. Assist with admissions, transfers, and discharge planning. Coordinate outpatient appointments and on-site ancillary services, including optometry, podiatry, dental, and psychiatric services. Connect residents and families with community resources, healthcare agencies, and financial assistance programs. Program Support Assist in planning, implementing, and evaluating social service programs. Participate in policy development, review, and implementation. Support orientation and in-service training programs related to residents' social, emotional, and medical needs. Participate in quality improvement initiatives and interdisciplinary meetings. Compliance & Documentation Complete required reports, assessments, and documentation accurately and timely. Maintain confidentiality of resident information and protected health information (PHI). Ensure compliance with federal, state, and facility regulations. Report any known or suspected incidents of fraud, abuse, neglect, or unauthorized disclosure of resident information. Qualifications Experience in social services, healthcare, skilled nursing, case management, or a related field preferred. Knowledge of community resources, discharge planning, and resident advocacy is a plus. Strong communication, organizational, and interpersonal skills. Ability to work effectively with residents, families, healthcare professionals, and community agencies. Compassionate, professional, and committed to resident-centered care.
