Social Worker (Full-Time)– Rochester, NY

Responsibilities

Provide social work services to patients in the field Conduct assessments and develop care plans Collaborate with interdisciplinary teams to support patient care Maintain accurate and timely documentation Ensure compliance with all regulatory and company standards Qualifications: BSW or MSW (required) Experience in working with clients with TBI and developmental delays (required) Strong interpersonal and communication skills Ability to work independently and manage field-based responsibilities Compensation: Salary: $70,000–$74,000 annually (based on experience) Schedule: Monday–Friday, 9:00 AM–5:00 PM
