The Peer Wellness Coach mentors and supports participants in managing their mental and physical health through one-on-one coaching and group facilitation. They assist participants and families in navigating the healthcare system and advocating for appropriate medical care.
Candidates must have lived experience with mental illness or substance use disorders and a high school diploma or GED. A Certified Peer Specialist certification is required or must be obtained within one year of hire.
SUMMARY OF POSITION The Peer Wellness Coach works with participants to mentor, encourage, support and problem-solve difficulties they may have in managing their mental and physical health and achieving improved health status. Duties include facilitating wellness groups using self-management care models, as well as one-on-one wellness coaching, assisting family/caretakers and participants in learning how to advocate for themselves, insuring participants access medical care which could include transporting to medical providers, and assisting with public presentations, community education. The Peer Wellness Coach is required to adhere to the Certified Peer Recovery Specialist Code of Ethics. ESSENTIAL DUTIES & RESPONSIBILITIES Assists participants and their families/caretakers in developing self-management skills to improve their health by teaching disease management strategies Provides wellness coaching, assisting family/caretakers and participants to learn how to advocate for themselves so they can independently access medical care that is most appropriate to treat their symptoms. Facilitates wellness and prevention groups using evidenced based and promising practices self-management models, consistently practicing faithfulness to the model Ensures participants understand how to successfully navigate the health care system by providing education and support to them and their family/caregivers Educates consumers using personal health information and provides education to assist them to use the information for self-management Ensures participant`s care is coordinated by collaborating and actively communicating with service providers and along with the participant develops strategies to remove barriers; allowing them to achieve optimal health status as they engage in the health care KNOWLEDGE, SKILLS & ABILITIES Effectively communicate via written, verbal, in person and virtual methods. Must be a person with lived experience with mental illness, substance use disorders and/or co-occurring disorders. Must demonstrate a personal commitment to a healthy lifestyle and preferred background in wellness promotion. Experience in providing social and recreation activities are desirable. Experience serving people with mental illness and/or co-occurring disorders is desirable. Must possess excellent organizational, problem-solving, and communications/listening skills. Must possess the ability and desire to learn and practice new skills including strength-based interventions; and positive reinforcement. Familiarity with MS Office applications and use of the internet required. Must value cultural differences, believe in the principles of recovery and be able to work independently and as a team member, problem-solve, and motivate others. QUALIFICATIONS Education Level High school diploma or GED required. Years of Experience Experience in providing social and recreation activities is preferred. Experience serving people with mental illness and/or co-occurring disorders is preferred.
