Act as a liaison for Medicaid members to ensure access to appropriate care through home and social assessments. Educate members on health wellness, preventative measures, and the appropriate utilization of healthcare services.
Requires a High School Diploma or GED and at least 2 years of residency in the local community. Must be able to travel locally up to 60% of the time and possess intermediate PC proficiency.

Bachelor's degree in social work, licensed practical nurse (LPN), CNA/HHA or HS graduate with 3 years of experience working within the community health setting in a health care role.
