Required Qualifications

High School Diploma / GED Lived in the local community for 2+ years Must have a designated workspace inside the home with access to high - speed internet availability Intermediate level of proficiency navigating a PC to open applications, send emails, and conduct data entry Intermediate level of knowledge of culture and values of the community with familiarity with the resources available in the community Reside in Kentucky Ability to travel locally up to 60%25 of the time

Preferred Qualifications

Bachelor's degree in social work, licensed practical nurse (LPN), CNA/HHA or HS graduate with 3 years of experience working within the community health setting in a health care role. Knowledge and / or experience with behavioral health or substance use disorders 2+ years of field - based experience 2+ years working in a case management role Previous care management experience CHW Certification Experience working in Managed Care Knowledge of Medicaid and / or Medicare population Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
