Provide skilled occupational therapy services in patients' homes under the direction of an OT and physician to restore independence and improve function. Educate patients and families on treatment plans and the safe use of adaptive equipment while maintaining accurate clinical documentation.
Must hold a current Occupational Therapy Assistant (OTA) license in the state of employment and have at least one year of acute care experience. A valid driver's license and current auto insurance are required.

You'll educate patients and their families, promote safety, and support progress toward treatment goals. You will also be responsible for maintaining accurate clinical documentation and ensuring compliance with all applicable legal, regulatory, and agency requirements. Key Responsibilities Plan of Care Provide skilled occupational therapy services under the direction of the Occupational Therapist and attending physician. Observe and monitor patient conditions. Report changes in patient status to the Occupational Therapist. Patient Care & Risk Reduction Assess functional limitations and provide appropriate interventions. Collaborate with the Occupational Therapist to achieve treatment goals and improve patient independence and functional status. Patient & Family Education Educate patients and caregivers on disease processes, injuries, treatment plans, and home management strategies. Instruct patients and family members on the safe use of assistive devices and adaptive equipment, including: Wheelchairs Walkers Canes Crutches Braces Prosthetic and orthotic devices Documentation & Compliance Complete all clinical documentation accurately and in a timely manner. Follow agency policies, Medicare guidelines, and federal regulations. Professional Responsibilities Adhere to agency policies, procedures, and regulatory requirements. Communicate schedule or availability changes to supervisors and scheduling staff. Attend required in-service trainings and agency meetings.
