  • Agency: Summa Health at Home and Hospice How You'll Make a Difference Care and compassion are at the heart of what you'll do as an Occupational Therapy Assistant (OTA). As part of an interdisciplinary care team, you'll help deliver high-quality, patient-centered care that promotes positive outcomes and supports patients throughout their recovery journey. Working under the direction of the attending physician and following treatment plans established by the Occupational Therapist (OT), you'll provide skilled occupational therapy services in patients' homes. You'll also collaborate closely with patients and their families, offering education, support, and guidance every step of the way. Home health care provides a unique opportunity to see patients in their everyday environments, allowing you to make meaningful recommendations that help restore independence, improve function, and enhance quality of life.

You'll educate patients and their families, promote safety, and support progress toward treatment goals. You will also be responsible for maintaining accurate clinical documentation and ensuring compliance with all applicable legal, regulatory, and agency requirements. Key Responsibilities Plan of Care Provide skilled occupational therapy services under the direction of the Occupational Therapist and attending physician. Observe and monitor patient conditions. Report changes in patient status to the Occupational Therapist. Patient Care & Risk Reduction Assess functional limitations and provide appropriate interventions. Collaborate with the Occupational Therapist to achieve treatment goals and improve patient independence and functional status. Patient & Family Education Educate patients and caregivers on disease processes, injuries, treatment plans, and home management strategies. Instruct patients and family members on the safe use of assistive devices and adaptive equipment, including: Wheelchairs Walkers Canes Crutches Braces Prosthetic and orthotic devices Documentation & Compliance Complete all clinical documentation accurately and in a timely manner. Follow agency policies, Medicare guidelines, and federal regulations. Professional Responsibilities Adhere to agency policies, procedures, and regulatory requirements. Communicate schedule or availability changes to supervisors and scheduling staff. Attend required in-service trainings and agency meetings.
