Manage a panel of post-acute patients across skilled nursing facilities, overseeing admissions, chronic disease management, and acute illness. Coordinate care transitions and collaborate with interdisciplinary teams to deliver patient-centered primary care.
Requires a Master's degree or higher in Nursing or a PA program with board certification (FNP, AGPCNP, or PA-C). Must hold an active California license, DEA registration, and have at least 6 months of clinical experience.

We're looking for a Nurse Practitioner or Physician Assistant who thrives in an autonomous, fast-paced clinical environment and genuinely enjoys the complexity of the post-acute patient. This is a SNFist-model role, meaning you'll carry your own panel of patients across one or more skilled nursing facilities, managing everything from admission assessments and chronic disease management to acute illness and care transitions.
