Provide high-quality nursing care, assessments, and complex treatments to patients in their homes to prevent hospital readmissions. Coordinate care across disciplines and maintain accurate documentation of patient conditions and interventions.
Requires a nursing degree, current RN licensure, BLS certification, and a valid driver's license with insurance. Candidates must have at least one year of clinical RN experience in an acute medical/surgical or homecare setting.
