About the role

Provide triage support using embedded protocols and resources to establish and evaluate patient plans of care. Coordinate ongoing support for patients and families while performing comprehensive assessments and providing health education.

HealthPartners North Suburban Family Physicians has an exciting opportunity for a Triage RN to join our team! This .80 fte position will be based at our Hugo clinic with potential for coverage at our Roseville or Lino Lakes clinics.

This position will work in clinic Monday, Tuesday, Thursday, Friday days (start time between 7:45am – 8:30am and end time between 4:15pm – 5:30pm).

This position provides triage support for our clinic at the North Suburban Family Physicians clinics by using protocols embedded in the electronic medical record and a variety of resources including standing orders and protocols. This position also includes occasionally working with patients that have questions or needs once on-site. In addition, this role provides comprehensive assessments to establish and evaluate a plan of care and address changes in a patient’s condition, delegates nursing tasks when appropriate and provides supervision and monitoring of function or activity, creates and implements dynamic teaching plans based upon identified patient need and acts as a resource for other staff.

Required Qualifications

1. Graduate of an accredited school of nursing

2. Current RN licensure in Minnesota

3. Current RN licensure in Wisconsin (within two months of hire)

4. Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained. On-line BLS certification is not acceptable.

5. Strongly demonstrated written and oral communication skills

Preferred Qualifications

  • One-year full-time employment as an RN
  • Triage or Care Management experience within the last three years

Uses health coaching techniques to elicit patient goals and priorities to develop individualized patient action plans

Uses population management tools such as registries to identify high risk patients and track, monitor and follow-up with their care

Engages, assesses readiness to learn and educates patients and families to implement strategies to reach a desired goal or care intervention

Identifies and secures patient services needed for patient care

Informs and supports patients and families to assist them in making informed decisions and navigate the healthcare system.

Education

Education imparts knowledge designed to develop skills, to enhance decision-making, and to promote understanding. It includes interaction with the patient, family and community, as well as participating in formal and informal professional education,

Requirements

Must be a graduate of an accredited nursing school with current RN licensure in Minnesota and BLS certification. Preferred candidates have one year of RN experience and recent triage or care management expertise.
