Physical Therapist - PT PRN

As a Home Health Physical Therapist, you’ll practice at the top of your license while delivering one-on-one rehabilitative care directly in patients’ homes. In this role, you will assess, plan, organize, and implement individualized therapy programs that improve mobility, reduce pain, restore function, increase strength, and help patients maintain independence and quality of life.

You’ll partner with patients, caregivers, and an interdisciplinary care team to create meaningful outcomes through compassionate, evidence-based care.

As a physical therapist, you will

• Perform and document comprehensive initial evaluations to identify patient needs and establish a clinical diagnosis prior to intervention.
• Develop, implement, and modify individualized treatment plans to maintain, improve, or restore physical functioning.
• Deliver therapeutic interventions focused on reducing pain, improving mobility, preventing disability, and maximizing patient outcomes.
• Evaluate patient response to treatment and adjust care plans throughout the episode of care to achieve optimal results.
• Apply infection control and universal precaution standards during all patient interactions.
• Maintain continuity and quality of patient care through timely, accurate documentation.
• Communicate effectively with patients, families, physicians, and interdisciplinary team members.
• Support and promote agency mission, values, and operational policies.
• Complete all documentation within established company timelines and guidelines.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and unrestricted Physical Therapist license in the applicable state.
• Minimum of one (1) year of Home Health experience preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation with current auto insurance.
• Experience with HomeCare HomeBase preferred.

Physical demands

• Ability to stand, walk, sit, climb, crawl, squat, and kneel for extended periods.
• Ability to lift, carry, push, and pull up to 100 lbs.
