The ACT Registered Nurse monitors all medical and clinical aspects of services for adults with severe mental illness. Responsibilities include coordinating care with providers, managing medication adherence, and delivering psychopharmacologic services within a multidisciplinary team.
Candidates must hold a Nursing Degree from an accredited school and a current North Carolina Registered Nurse license. A valid driver's license, reliable transportation, and two years of experience with behavioral health clients are preferred.
Are you a compassionate nurse who is passionate about making a difference beyond traditional healthcare settings? As an Assertive Community Treatment (ACT) Registered Nurse, you will work alongside a multidisciplinary team to provide comprehensive, community-based care to adults living with severe and persistent mental illness. You will help individuals manage their physical and behavioral health needs, support medication adherence, coordinate care across providers, and serve as a key member of a team dedicated to promoting recovery, independence, and improved quality of life. The ACT Nurse will be in charge of monitoring all medical/clinical aspects of the services delivered. Overall monitoring of all medical/clinical aspects of the services delivered. Identify appropriate evidence-based or emerging best practices therapies to be employed by staff in delivering services. Consult with primary healthcare providers/other providers on the clinical aspects of the clients being served and the implications for those clients' physical healthcare needs. Coordinate with medical staff of other providers, e.g., state and community inpatient facilities. Assist in identifying, planning, scheduling, and implementing services for the client. Assist in the development of transition plans when clients move in and out of services. Provide any written documentation as required. This will include treatment and goal plans and activity notes for the medical record (e.g., service notes, etc.) This description outlines the general scope and level of work for this role. Compensation This role is compensated on a salaried basis, providing consistent earnings aligned with the responsibilities and expectations of the position.
