$55,000 - $60,000+ Incentive Bonuses – SPARC Services and Programs is seeking an Intake Coordinator to join our Family Centered Treatment (FCT) team in the Statesville, NC area.

About the Role

The FCT Intake Coordinator manages the referral and intake process for assigned FCT teams, ensuring timely, accurate, and compliant onboarding of new clients. This role bridges clinical and administrative functions, coordinating closely with the Business Office, FCT Supervisor, and Clinical Team to support quality service delivery from first contact through authorization.

Responsibilities

Coordinate referrals for services on assigned teams as forwarded by the Business Office, in collaboration with the FCT Supervisor Coordinate and schedule intakes promptly and accurately Complete all required intake documentation, including Comprehensive Clinical Assessments (CCAs), Person-Centered Plans (PCPs), Crisis Plans, and other required forms; complete consumer paperwork related to agency orientation, policies, and procedures Submit all initial authorization requests to the applicable payer or Tailored Plan, including Standard Plan Change Requests and required follow-up Coordinate with the Business Office to provide documentation needed to add authorizations to the EHR Consult with approved licensed professionals regarding medical necessity for appropriate services Provide clinical consultation and recommendations to assigned FCT teams in collaboration with the FCT Supervisor and Clinical Team Attend weekly FCT Team Meetings Provide field support as requested by the FCT Team or Supervisor In smaller geographic service areas, the role may blend intake coordination with direct FCT service delivery; assigned caseload will be proportional to intake coverage needs and regional operational demands Represent SPARC on external committees as assigned by the FCT Supervisor Implement program policies and procedures Participate in required in-service trainings Support quality client services through participation in the agency's continuous quality improvement process Demonstrate competence and responsiveness to cultural differences among both clients and staff Consistently uphold SPARC's values Perform other duties as assigned by the FCT Supervisor or VP of Operations

Qualifications

Master's degree in Human Services, Social Work, or Marriage and Family Therapy Licensed or provisionally licensed mental health professional (fully licensed clinicians preferred) Ability to engage clients and build rapport Knowledge of community resources Ability to work effectively with a culturally diverse population Ability to make sound judgment calls and quick decisions in crisis situations Strong oral and written communication skills Excellent organizational skills and ability to work independently Ability to meet training and documentation deadlines Intermediate to advanced computer/word processing skills Ability to work flexible hours, including carrying and responding to a company cell phone daily Ability to travel as necessary Ability to work well in a team environment Ability to conceptualize, organize, and follow through on assigned projects Current CPR and First Aid certification (or ability to obtain) Understanding of confidentiality requirements and regulations Valid driver's license, reliable personal vehicle, and valid auto insurance Must meet criminal background check requirements per service provision standards Join a diverse, inclusive, and whole-person focused team! Requirements
