About the role

Deliver professional nursing care to patients under the direction of a Registered Nurse. Coordinate home care services and communicate patient status changes according to agency guidelines.

We are seeking a part-time Licensed Practical Nurse to join our team! You will be responsible for delivering high-quality care to assigned patients. With towns including but not limited to Porter County, Indiana.

Qualifications

  • Graduate of an accredited school of professional nursing.
  • Current license to practice as a Licensed Practical Nurse in the state(s) of employment.
  • Minimum of one (1) year experience in an acute care setting or equivalent experience. Community/Home Health experience preferred.
  • CPR certified.
  • Demonstrated strong and effective written and verbal communication skills and good interpersonal skills.
  • Demonstrated experience with EMR system. Ability to use a tablet for documentation purposes and communication via email/text.
  • Ability to deal tactfully with clients and the community.
  • Demonstrated organizational and time management skills.
  • Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Has a good driving record.

Responsibilities

  • Provides professional nursing care as defined in the nurse practice act and under the direction of a Registered Nurse. Communicates appropriate status/condition changes to the RN per agency guidelines.
  • Promotes personal safety and a safe environment for clients and coworkers.
  • Performs job in compliance with Agency policies and procedures and professional and community standards.
  • Participates in coordination of home care services, attends meetings as required, and follows reporting guidelines.
  • Demonstrates basic understanding and proficiency of basic Glucometer and INR machines.
  • Demonstrates basic understanding and proficiency of wound care.
  • Performs other job duties as assigned

Requirements

Must be a graduate of an accredited nursing school with a current LPN license and at least one year of acute care experience. Requires CPR certification, a valid driver's license, and proficiency with EMR systems and tablets.
