About the Role
We are seeking a part-time licensed Occupational Therapist (OT) to join our exceptional team in Indiana. We have openings in Lake County (Crown Point, Hobart, Gary, Merrillville, Munster, etc.), Porter County (Valparaiso, Portage, Porter, Boone Grove, South Haven, etc.), and the surrounding areas.
What You'll Do Conduct comprehensive in-home occupational therapy evaluations and deliver targeted treatment interventions Develop and implement individualized care plans designed to maximize patient independence and functional outcomes Educate patients and caregivers on safety practices, adaptive techniques, and proper use of assistive equipment Complete accurate, timely, and compliant clinical documentation for all patient visits Collaborate effectively with the interdisciplinary team to communicate patient progress and adjust care plans as needed Ensure adherence to all clinical, regulatory, and documentation standards
What You Bring Active licensure as an Occupational Therapist (OT) in good standing Prior home health experience preferred; new candidates with strong clinical skills encouraged to apply Excellent communication and clinical documentation abilities Demonstrated ability to work independently while effectively managing a flexible schedule Reliable, professional, and committed to delivering patient-centered care

Requirements
Requires an active license as an Occupational Therapist in good standing. Prior home health experience is preferred, along with strong communication and independent time management skills.
