One Home Health is seeking an experienced home health aide/ CNA to support our Illinois team in a part-time capacity in McHenry County.
What You'll Do Deliver compassionate, individualized personal care to patients in their homes Assist with daily living activities, including bathing, dressing, grooming, and mobility Observe, document, and report any changes in patient condition to the care team Perform basic health-related tasks such as medication reminders and simple wound care, as permitted Promote patient comfort, safety, and overall well-being Provide guidance and education to family members on proper care techniques when appropriate Follow established care plans, agency policies, and regulatory guidelines Maintain patient confidentiality while ensuring a clean, safe, and secure home environment
What You Bring Certified Nursing Assistant (CNA) credential in good standing Experience in home health, hospice, or a related caregiving setting preferred Demonstrated compassionate, patient-centered approach to care Reliable, professional, and dependable with a strong work ethic Ability to perform the physical demands of caregiving, including lifting, transferring, and assisting patients Excellent communication and interpersonal skills
