Licensed Practical Nurse (LPN)

We are seeking a Licensed Practical Nurse to join our team! You will be responsible for delivering high-quality care to assigned patients. With towns including but not limited to Lake County Indiana.

Qualifications

Graduate of an accredited school of professional nursing. Current license to practice as a Licensed Practical Nurse in the state(s) of employment. Minimum of one (1) year experience in an acute care setting or equivalent experience. Community/Home Health experience preferred. CPR certified. Demonstrated strong and effective written and verbal communication skills and good interpersonal skills. Demonstrated experience with EMR system. Ability to use a tablet for documentation purposes and communication via email/text. Ability to deal tactfully with clients and the community. Demonstrated organizational and time management skills. Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Has a good driving record.

Responsibilities

Provides professional nursing care as defined in the nurse practice act and under the direction of a Registered Nurse. Communicates appropriate status/condition changes to the RN per agency guidelines. Promotes personal safety and a safe environment for clients and coworkers. Performs job in compliance with Agency policies and procedures and professional and community standards. Participates in coordination of home care services, attends meetings as required, and follows reporting guidelines. Demonstrates basic understanding and proficiency of basic Glucometer and INR machines. Demonstrates basic understanding and proficiency of wound care. Performs other job duties as assigned
