Job Overview

Fairview has an immediate opening for a Licensed Practical Nurse to join our Specialty team in Maplewood, MN.

You won't be bored of doing the same things day in and day out because you get to do different things in different specialties!

This is a 0.01 FTE (casual on-call position), non-benefits-eligible opening.

Clinic Hours of Operation are Monday-Friday 8am-5p.

No Weekends!

This position will require flexibility to float to all 3 regions (East region: Maplewood, Woodbury, Wyoming.

Edina). The LPN performs delegated nursing and medical functions of data collection and delivery of care to an assigned group of patients, or individualized patient. The LPN works under the direction of a Registered Nurse, or physician/authorized practitioner and in collaboration with other members of the healthcare team. The LPN is responsible for adhering to all policies, procedures, and nursing practice guidelines.

Responsibilities

Collects data for patient assessments. Involves the patient, family, and other health team members in gathering relevant data. Document’s data collected in the medical record. Reports unusual data or changes in patient condition to the RN, or physician/authorized practitioner in a timely manner. Delivers care as delegated by the RN or physician/authorized practitioner to assigned patient population, or individual patient. Follows the patient’s plan of care. Prioritizes work based on patient acuity, available resources, and patient/family preferences, in collaboration with the RN or physician/authorized practitioner. Performs high and low risk technical procedures, including medications, defined by the clinical setting. Recognizes when different approaches may be needed based on age or cultural differences. Implements assigned aspects of the patient teaching plan. Documents care in the medical record. Effectively communicates compliance and concerns to delegating RN or physician/authorized practitioner. Takes action to assure continuity of care for all assigned patients. Adheres to all relevant policies, procedures, and nursing practice guidelines. Responds appropriately to emergent situations. Communicates effectively with other members of the health care team. Documents care in the medical record. Communicates verbal and written information in a clear, concise, accurate, and timely manner. Uses communication approaches appropriate to patient/family needs. Required Qualifications Vocational School or Associates Degree in Nursing MN Licensed Practical Nurse (LPN)

Preferred Qualifications

1 year Previous ambulatory Care experience. Previous wound care experience. Basic Life Support (American Heart Assoc or Red Cross)

Requirements

Candidates must have a Vocational School or Associate Degree in Nursing and a valid MN Licensed Practical Nurse (LPN) license. Preferred qualifications include one year of ambulatory or wound care experience and BLS certification.
