The Intake Coordinator is responsible for screening and conducting intakes for individuals entering the PROS Program. They perform assessments to develop Individual Recovery Plans and coordinate with referral sources and collaterals.
RiseWell Community Services is seeking a compassionate Full-Time Intake Coordinator to join our PROS Program- Personalized Recovery Oriented Services in Patchogue, NY
In this role, you will be responsible for screening and conducting intakes for individuals seeking admission to the PROS Program. Conducts assessments to determine individual’s needs and preferences and develops recommendation for the person’s Individual Recovery Plan. Conducts new member orientation and coordinates with all referral sources and collaterals. Maintains appropriate records according to program regulations
Licensed Master Social Worker. Experience working with individuals in recovery from Mental Illness. Valid and clean NYS Drivers License. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

Candidates must be a Licensed Master Social Worker with experience working with individuals recovering from mental illness. A valid NYS Driver's License and approval from the NYS Office of Mental Health are required.
