The Hospice RN Case Manager coordinates comprehensive care for terminally ill patients through assessment, personalized care planning, and symptom management. They serve as the primary contact for families and collaborate with an interdisciplinary team to provide holistic end-of-life support.
Patient Assessment and Care Planning (25%) Conduct initial and ongoing assessments to evaluate patients’ physical, emotional, and spiritual needs. Develop personalized care plans in collaboration with the patient, family, and interdisciplinary team. Review and update care plans based on changes in patient condition and goals of care. Care Coordination (20%) Serve as the primary point of contact for patients, families, and caregivers. Coordinate services and resources to ensure continuity of care. Facilitate communication among healthcare providers and community agencies involved in patient care. Clinical Documentation, Reporting and Team Collaboration (15%) Maintain accurate and up-to-date clinical records, including patient assessments, care plans, and progress notes. Current RN license in the state of practice. Certification in Hospice and Palliative Care Nursing (CHPN) preferred. Must possess and maintain current CPR certification.
Previous experience in hospice or palliative care preferred.
