Let's talk about the Care Manager position!

This full-time, hybrid position combines remote work from your home office with travel of up to one hour for community-based meetings several times a week. To accommodate required in-person meetings, candidates must reside within Indiana or within 10 miles of the state line.

What You’ll Do

  • Provide information and education to help Individuals understand the Medicaid waiver process
  • Create an individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
  • Help Individuals access services and supports they need to meet their goals
  • Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Who You’ll Work With

Case Managers work with Individuals on the Health & Wellness (H&W) and Traumatic Brain Injury (TBI) Medicaid Waivers

What You Need

  • A valid driver's license, car insurance, and reliable transportation
  • RN OR bachelor’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health & Human Services OR any field with 2 years full-time, direct service experience with the elderly or disabled OR master’s degree in social work, Psychology, Counseling, Gerontology, Nursing or Health and Human Services
  • The ability to pass thorough background screens
  • Designated home office setting with computer, internet service, and smart phone

Requirements

Requires a bachelor's or master's degree in a human services field, or an RN license, or 2 years of direct service experience with elderly or disabled populations. Must have a valid driver's license, reliable transportation, and a designated home office setup.
