About the role

The Triage Nurse provides telephone support to hospice patients and families by assessing needs and determining appropriate interventions. They coordinate hospice services, communicate with physicians, and maintain accurate electronic documentation.

Overview

Good Shepherd Hospice is seeking a compassionate and experienced Registered Nurse (RN) to provide telephone triage support for hospice patients and families. The Triage Nurse assesses patient needs, coordinates care and services, provides clinical guidance, and ensures timely interventions to support quality end-of-life care.

Job Details

Key Responsibilities Respond to patient, family, and caregiver calls during assigned triage hours.

Assess patient needs and determine appropriate interventions.

Coordinate hospice services, including medications, equipment, supplies, and nursing visits.

Communicate with physicians and interdisciplinary team members regarding patient status and care needs.

Provide education, support, and reassurance to patients and families.

Complete accurate and timely electronic documentation.

Participate in staff meetings, continuing education, and team collaboration.

Qualifications

Graduate of an accredited nursing program; BSN preferred. Current New York State RN license required. Minimum of 2 years of recent experience in hospice, oncology, home health, or medical-surgical nursing. Hospice and palliative care certification preferred. Strong clinical assessment, communication, and computer skills

Salary Range

Usd $50.00 - Usd $60.00 /Hr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits. Requirements

Candidates must be a graduate of an accredited nursing program with a current New York State RN license. A minimum of 2 years of experience in hospice, oncology, home health, or medical-surgical nursing is required.
