Qualifications

Certificates, Licenses, Registrations: Valid and current dental license, DDS or DMD Degree from an accredited dental program Active CPR/BLS certification Active DEA certification Active pediatric minimal sedation (PMS) permit Active NPI number

Requirements

Requires a DDS or DMD degree from an accredited program and a valid dental license. Must hold active CPR/BLS, DEA, NPI certifications, and a pediatric minimal sedation (PMS) permit.
