About the role

The nurse is responsible for the assessment, management, and treatment of pediatric patients through home visits and quarterly evaluations. Key duties include writing Plans of Care, maintaining clinical notes, and providing education to patients and their families.

Registered Nurse

We are seeking Registered Nurses to join our Team! You will be responsible for the assessment, management, and treatment of assigned patients.

Responsibilities

Include:

· Visit clients in the privacy of their residence in designated geographic territories.

· Schedule and preform monthly supervisory visits

· Schedule and preform Quarterly Evaluations

· Complete timely and accurate clinical notes including addressing patient's progress.

· Evaluate the client status and the skills of workers while the staff member is present.

· Keeps documentation of all communication with clients, family members and support staff. .

· Provides patient, family, and/or caregiver education and information pertinent to diagnosis and safety issues.

· Counsels and educates the patient and family in meeting healthcare and related needs.

· Write 485/ Plan of Care for patients when reauthorizations are due for patients.

· Other projects/duties as assigned.

Requirements

  • · Home Care experience preferred
  • · Excellent verbal and written communication skills.
  • · Highly motivated, organizational, leadership and relationship building skills
  • · Ability to work in a very detailed, fast paced environment
  • · Currently holds a RN license in PA
  • · Valid state driver’s license and car insurance
